Everything you need to know about the CMS GUIDE model and how Ohio organizations can get involved
This 30-minute presentation, followed by a Q&A, will include an overview of CMS' goals in developing GUIDE, a review of the nine required elements of GUIDE delivery—including clinical care, care navigation, respite, and other services—participant eligibility parameters, and the unique role that providers of older-adult services and health care organizations can play in the process.
You'll come away with a clear understanding of what the Ohio landscape will look like. More importantly, you'll learn the role that area agencies on aging (AAAs), community-based organizations, and health providers can play in referring eligible GUIDE participants to the program and receiving referrals that will increase usage of your current programming.
Benjamin Rose Institute on Aging is an active participant in GUIDE service delivery through our evidence-based BRI Care Consultation program. We've partnered with several health providers to deliver the care navigation elements of the GUIDE program.
