CEHE Background Check Requirement Instructions
The CEHE background check can be completed in one of two ways:
- If the student is placed in a practicum experience at his/her employer, the student may submit a letter from the employer’s Human Resource department to edprep@osu.edu.
- The letter must be on their letterhead from someone with authority (usually this is HR).
- The letter must contain information on which background checks were completed and the expiration dates.
- The employer must indicate that the background checks satisfy their requirements for the placement.
OR
- Complete the BCI/FBI background check at the Office of Human Resources. The following codes must be used to fulfill this requirement:
- BCI background check with the code of 3319.291
- FBI background check with the code of 3319.291
- Complete the WebCheck Consent Form via DocuSign. The online consent form must be completed before your scheduled appointment.
Ohio students who are unable to utilize the Ohio State Office of Human Resources services can find an approved fingerprinting agency in their area here. Results must be sent to the following campus address:

- If you plan to apply for licensure within the year, you must request a copy of the background checks be sent to the Ohio Department of Education. You can indicate this as your preference on the above form.
- Background checks are valid for one calendar year from the date they are completed.
If a student is seeking licensure at the completion of the certificate, the Ohio Department of Education will require all applicants to have a current background check (within one year) on file with their office.
