RN Licensure Submission Instructions
Information pertaining to obtaining RN licensure and documentation for submitting.
As a student in one of the following programs listed below, you are required to submit documentation from the state board of nursing licensure verification website for all RN licenses you possess, including the state where you will be completing coursework.
The following types of students must have an active, unencumbered RN License:
- Second Year GE students
- PhD
- DNP
- DNE
- Traditional Master's
- Post Master's
- RN to BSN
Submitting state board documentation
Documentation must include all of the following information:
- Your full name
- License number
- Expiration date
- State of authorization
- Board action status
If you are experiencing issues with submission, select the PDF file you wish to upload, and wait for the name of the file to appear prior to clicking “submit”.
Ohio license sample documentation
RN License verification should include your name, RN License number, state of licensure, expiration date and no board actions or restrictions. This can be found at the online licensure verification webpage for your state of licensure.
