Apply to Graduate
Students in the College of Nursing must meet all university graduation requirements, which varies by program. Find your program below and follow all steps.
Undergraduate Students
Undergraduate students in the College of Nursing must meet all university graduation requirements. You may apply to graduate one term after entering your major, but no later than the 10th Friday of the semester you plan to graduate. If your graduation date changes after applying, you must submit a new application with the updated date.
Select your program for application instructions
- Apply to graduate
- You must submit before the 10th Friday of the term in which you are applying to graduate.
- Per University rules: If the deadline is missed, you must apply to graduate for the next term.
- Enter the information on the form accurately. If a field does not apply, then leave it blank (e.g., middle name, minor, honors)
- Processing
- Allow 2-3 days for your graduation status to show up in Buckeye Link.
- If applied in autumn, it may take up to two weeks for your app to graduate to be processed due to the high volume of applicants.
- Check your status in Buckeye Link
- Select “My Academics” to the left of your Student Center and View my Graduation Status” in the Graduation box
- You should see your expected Graduation Term as updated to the correct term and your diploma name listed as you indicated.
Graduate Students
Graduate students in the College of Nursing must meet all requirements set by The Ohio State University Graduate School. Prior to applying to graduate, review the Graduate School handbook for the requirements set for doctoral and master level students. You may apply to graduate up to one semester before your intended term, but no later than the second Friday of that semester. The College of Nursing shortens the Graduate School’s deadline by one week to allow time for document processing.
After submitting the application through Grad Forms, you should review its status to ensure the processing and approval of the application:
- Initial Review: indicates the application is awaiting action from a member of the College of Nursing, typically the faculty advisor. Please contact your faculty advisor in the College of Nursing if your status appears as “Initial Review” for more than a few days after submission.
- Graduate School Review: indicates the application has been approved by the faculty advisor and the form is awaiting final processing by the Graduate School.
Final approval is granted by the Graduate School after the deadline to apply to graduate, which may occur midterm.
Select your program for application instructions
- Log into Grad Forms using your Ohio State username and password
- Select Graduation and Examination Forms, locate the Application to Graduate and click on the green + Create New Application to Graduate button.
- Select the term you plan to graduate
- Do not check the End of Semester box unless specifically instructed.
- Select the appropriate Graduation Level
- Professional, Graduation Plan: NURBSN-DNP and Gradation Degree: DNP and for End of Semester Option = No (do not change).
- Complete Commencement section
- If you authorize someone to pick-up your diploma in your place, put their full legal name in the space provided.
- If you plan to have the diploma mailed to your permanent address, confirm the address that appears once you select this option. If the address is not correct, visit your student center through Buckeye Link and make the necessary corrections.
- Complete Committee Section
- Type in your advisor’s last name and wait for a drop-down menu to appear and click on it. Repeat this process for all “Committee Members”. DNP students should submit the names of their final project committee.
- If you have an external committee member who is not part of the Ohio State Graduate Faculty, follow these steps to complete the Committee Section:
- Input the person’s name and email address in the spaces provided under External Members and click Add. If you have more than one external member, repeat this process as needed.
- In addition to submitting the external committee members on this form, you must also submit the Committee and Examination Petition form through Grad Forms for each external committee member. External committee members do not count towards the minimum number of required graduate faculty committee members, as they are included in addition to these requirements for a valid committee.
- Save and submit your application
- Save your application for further review and discussion, or click Save and Continue for the next steps. Note: The application must still be submitted after this step.
- Next click Submit. Note: Be sure to submit your application to graduate early in order to allow sufficient time for your faculty advisor and program to review and act on your application prior to the deadline.
- Review of your application
- Approval of your form only places your name on the Graduation List and is not a guarantee that you will graduate.
- If the designation/status = Pending, it is awaiting action by someone in the College of Nursing (usually an advisor).
- Your application will be reviewed by the Graduate School after the deadline to apply to graduate has passed, and will remain as "Pending GS" (pending Graduate School approval) until this time.
- If there is anything incorrect regarding your application, you will receive notification from the Student Data Manager.

- Select the term you plan to graduate.
- Do not check the End of Semester box unless specifically instructed.
- Select the appropriate Graduation Level
- Master’s Non-Thesis, Graduation Plan: NURSING-MS, Graduation Degree: MS and End of Semester Option = No (do not change).
- Complete Commencement section
- If you authorize someone to pick-up your diploma in your place, put their full legal name in the space provided.
- If you plan to have the diploma mailed to your permanent address, confirm the address that appears once you select this option. If the address is not correct, visit your student center through Buckeye Link and make the necessary corrections.
- Complete Committee Section
- BSN to DNP students completing the Master’s portion of the program should refer to Beacon to view their advisor of record and assigned committee member (aka second reader). This information will be posted to Beacon on or around week eight of the semester preceding the term of your anticipated graduation date.
- Type in your advisor’s last name and once generated, click on it. Repeat this process for the additional Committee Member, which may differ from your current DNP committee. Only your advisor and the one committee member identified should be included.
- Save and submit your application
- You will receive an error message when submitting your application stating "WARNING: You are currently enrolled as a professional student and you did not select the professional graduation level." Disregard this warning and proceed with submitting your application.
- You can take the following actions on your application:
- Save your application for further review and discussion, or Save and Continue for the next steps. Note: The application must still be submitted after this step.
- After clicking Save and Continue click Submit. Note: Submit your application to graduate early in order to allow sufficient time for your faculty advisor and program to review and act on your application prior to the deadline.
- Review of your application
- Approval of your form only places your name on the Graduation List and is not a guarantee that you will graduate.
- If the designation/status = Pending, it is awaiting action by someone in the College of Nursing (usually an advisor).
- Your application will be reviewed by the Graduate School after the deadline to apply to graduate has passed, and will remain as Pending GS (pending Graduate School approval) until this time.
- If there is anything incorrect regarding your application, you will receive notification from the Student Data Manager.

- If you plan to have the diploma mailed to your permanent address, confirm the address that appears once you select this option. If the address is not correct, visit your student center through and make the necessary corrections.
- Complete Committee section
- Graduate Entry and Traditional Master’s students should refer to Beacon to view their advisor of record and assigned committee member (aka second reader). This information will be posted to Beacon on or around week eight of the semester preceding the term of your anticipated graduation date.
- Type in your advisor’s last name as shown on the list on Beacon and select their name. Repeat this process for the additional Committee Member.
- Save and submit your application
- Save your application for further review and discussion or click save and continue for the next steps. Note: The application must still be submitted after this step.
- Next click submit. Note: Submit your application to graduate early in order to allow sufficient time for your faculty advisor and program to review and act on your application prior to the deadline.
- Review of your application

- Select the completion term and certificate plan from the drop down list.
- List the courses that apply to the Certificate. This information is available on Beacon.
- Review the form for accuracy and then click save and continue.
- Next, click submit. Note: Allow sufficient time between submitting your application and the deadline by which it must be to the Graduate School. Your application will be reviewed once it is received by the Graduate School.
- Log into Grad Forms using your Ohio State username and password
- Select Graduation and Examination Forms, locate the Application to Graduate and click on the green + Create New Application to Graduate button.
- Select the term you plan to graduate
- Do not check the “End of Semester” box unless specifically instructed.
- Select Graduation Level
- Master’s – Course Based (you will not need to list your advisor or any committee members), Graduation Plan: CLNRSRC-MO, Gradation Degree: MCR and End of Semester Option = No (do not change).
- Complete Commencement section
- If you authorize someone to pick-up your diploma in your place, put their full legal name in the space provided.
- If you plan to have the diploma mailed to your permanent address, confirm the address that appears once you select this option. If the address is not correct, visit your student center through and make the necessary corrections.
- Save and submit your application
- Save your application for further review and discussion or click save and continue for next steps. Note: The application must still be submitted after this step.
- Next, click submit. Note: Submit your application to graduate early in order to allow sufficient time for your faculty advisor and program to review and act on your application prior to the deadline.

FAQ
This is not applicable to the Post-Master’s option.
- Log into Grad Forms, select “Search Forms” and click blue “Apply Filters” button.
- Locate the form on the auto-generated list. You can determine the status by looking at the “Approval Status” column.
- Pending: Your program or advisor still needs to act on the form, or the Graduate School has received the form by the deadline.
- Pending GS: The College of Nursing has processed and approved all documentation required for graduation. The approval from the Graduate School is still pending.
- Approved: The form has been approved by the Graduate School (this will occur after the deadline).
- Check the “History.”
- Click white “View” button in the far-right column.
- Click “View Application to Graduate” pop up button. When the form shows on your computer screen, you will see five tabs: Student Information, Form Name, History, Remarks and Comments.
- Click on the “History” tab to see who has or has not acted on the form or the “Comments” tab to see if there are any issues noted. It is your responsibility to follow up with your advisor prior to the deadline if they need to approve the form.
- Log into Grad Forms and select “Search Forms”, click the blue “Apply Filters” button. When the results from the search show up on your screen, scroll down the list and locate your application.
- Go to the far right column and click “View” and then the “View Application to Graduate” button. When your application comes on screen, click the blue “Actions” button and select “Cancel Application.”
Per university policy, students must be enrolled in an undergraduate or graduate degree program to complete an embedded certificate. Meaning a student cannot graduate from an undergraduate or graduate degree program and then return in a future semester to complete the embedded certificate program.
