Drug Screening Instructions
You are required to complete a drug screen by the posted deadline for your program. You will not be permitted to attend clinical or health assessment if results have not been received by the deadline for your program. If you cannot complete a drug screen at the Wilce Student Health Center you are responsible for seeking out a facility on your own and paying for the cost of the drug screen.
Instructions
You may use this form (page seven) to document the drug screening.

- For your appointment, bring all prescriptions of medications/supplements currently being taken in the case of a false positive result. If for any reason the drug screening must be completed again, whether due to a positive result or false positive result, you will incur the cost of the repeated test.

- Benzodiazepine
- Barbiturates
- Cocaine
- Ecstasy
